Go to a doctor. Seriously, if worrying about your health is making you fall apart, you need to get that taken care of. It's all about the hierarchy of needs, which says humans need to have their basic needs met before moving on to "higher" things. It's hard to concentrate on schoolwork or projects or relationships when you feel like you're about to collapse at any moment.
I have horrible sleep issues, and it really does impact my mental health. (I've been going to doctors for a long time to work on this, it's a work in progress.) At least go to a doctor and see if anything can be done about the physical issues, then you can concentrate on the mental/emotional.
