Picking a style you like (other than the fact that it doesn't have a header, background, whatever) and then customise it to have these things. I know very little about how coding a Livejournal layout works, but on the whole they seem to be quite customisable and there's plenty of help for this out there.
When you know what you want to do, go to one of the Livejournal communities dedicated to making and modifying layouts. This seems like a good place to start. If there's a special community for the style you chose, go there. (I use Flexible Squares for example, and got all my code from the community called s2flexisquares, which is also listed on that page.)
Then either look at the memories of the community, recent posts or make a post of your own if your subject isn't covered. (Again, to use myself as an example, I wanted to add an extra box to my sidebar and be able to use HTML with it, and s2flexisquares already had an entry like that in their memories. Convenient!) Adding a header or banner seems to be quite a popular thing to do, so there'll probably already be a tutorial for it.
Then just copy and paste the code from their tutorial into your own layout code, modify it according to the instructions and voilĂ . I still don't understand the S2 language, but got all the modifications I wanted for my layout done this way. ;-) Probably not ideal, but a possibility.
