Hey Wain,You did not install the exetnded version correctly. The new files have to go directly over the old ones as described in the directions.You will need to use an FTP program to clean up your WP plugins folder which according to that error message now contains a folder called:wordpress_simple_survey_unzip_firstThere can only be one version of the plugin installed and the folder has to be named: wordpress-simple-survey
