To start writing a program that extends the functionality of Windows XP with just the one feature that Vista has that improves upon Windows XP's start menu -
In vista, when you click on Start, you get an input field where you can type the name of a program, or a file, and it will go looking for it and show the results and you can then click it and open it.
If only that part could be added to the Windows XP start menu, then that'd be just fine by me.
I'd probably want to have it on the taskbar, though, so even clicking on the Start button is not necessary.
